I think it?s just small bug if you have played game long time straight, or just some bug with game code.

F.E. in Diablo II you must restart the game before you can enter hardest difficulty while playing online.

In the main directory of H&D2, where you can find the starting exes etc, there should be file called "setup.exe".

Have you ran that and checked all video stuff?

Well I have not seen K-19, but other possibilities could include the orchestra that made the musics might have been invovled with K-19 or they might have asked permission to use that music etc etc.

I found out myself that newest ForceWare (86.xx (dunno what the last numbers were) caused my H&D2 to have disappearing enemies and object past +20m distances.
